Altitude sickness: what to do
Descend immediately — don’t wait for morning
- unsteady gait — the person can’t walk a straight line
- confusion, odd behaviour, apathy
- breathlessness at rest that doesn’t ease when sitting
- wet cough, gurgling chest, pink sputum
- blue lips and nails
- vomiting that won’t stop
- headache that painkillers don’t touch
These are signs of cerebral or pulmonary oedema. The only real treatment is descent. Oxygen and drugs buy time but don’t replace going down. Tell the guide immediately — at night too.
Mild symptoms — safe to watch
Headache that responds to painkillers, nausea, poor sleep, breathlessness on exertion, no appetite. What to do: go no higher, drink a lot, walk more and lie down less, no alcohol. If it isn’t better by morning — descend.

Medical care along the route
Cusco — the best place on the route to treat altitude sickness
The private CIMA and Clínica Pardo clinics take foreigners, have English-speaking staff and hyperbaric chambers. A chamber session or two clears symptoms. This is the only point on the route with dedicated altitude medicine.
La Paz
Private clinics in Calacoto and Sopocachi. Better than public ones, but pricier. The city itself sits at 3,645 m — recovery here is slower than at low altitude.
Uyuni and Sur Lípez — the most exposed stretch
The town of Uyuni has only basic care; beyond it, along the 4x4 route, there is no medicine at all. The nearest serious hospital is in Potosí — hours away on dirt roads. Every 4x4 carries oxygen, but that is no substitute for evacuation.
San Pedro de Atacama
The village has a rural clinic with no full emergency care. Serious cases go to the Carlos Cisternas hospital in Calama, about 100 km away.
About evacuation
Air-ambulance evacuation from altitude can cost tens of thousands of dollars. That is exactly why the organizer’s recommendations require insurance covering altitude up to 5,500 m, medical evacuation and search & rescue. The receiving side’s evacuation protocol as of 13 Aug 2026 is not yet received — an open item in the documents.
